What Is A Standardized Patient?
A Standardized Patient (SP), also known as a simulated patient or simulated participant, is a person who is trained to act as a real patient to portray, in a consistent, standardized manner, a patient in a medical situation.
A Standardized Patient's job is to simulate situations for a student, such as:
- a medical history interview
- a victim of domestic abuse
- explaining medications
- give counseling about quitting smoking
- giving bad news to a loved one
- help nursing staff practice interpersonal skills
- help students practice navigating an exam room
- meeting a patient for the first time
- patients portraying angry or crying patients
- performing a physical exam
- play a grieving loved one
- play a hurricane victim for disaster preparedness
- play a mass-shooting victim for mass-casualty influx training
- the death of a child
You are expected to give the same standardized performance up to 12 times a day. This requires training, practice and hard work.
The use of Standardized Patients:
- allow students to practice their clinical and conversational skills before they work with real patients
- provide a safe environment to allow students to make mistakes and learn from them without doing any harm to real patients
- help train students to learn professional conduct in potentially embarrassing situations
- are used as part of an objective structured clinical examination, which tests a student's clinical skill performance and competence
How Much Do Standardized Patients Get Paid?
Standardized Patients typically earn $15 to $25+ per hour for work and training.
How Often Do Standardized Patients Work?
Standardized Patients are typically hired as a contractor, on an "as-needed" basis.
Most student sessions take place during weekdays, with the possibility of some weekend or evening work. A session may last from a few hours to all day.
Some Standardized Patients work "hundreds" of hours a year.
Is It Safe Being A Standardized Patient?
Yes. You are never asked to participate in situations which may make you uncomfortable..
Physical exams are very basic and do not cause any harm. At most, there might be some brief discomfort.
Encounters are videotaped and/or observed by a faculty person.
If you have any cause for concern, you have the right to stop any examination.
You will not be asked or expected to give blood samples or take any drugs.
What Is A Physical Exam?
If you have ever had a physical exam, then you will know what to expect.
In a physical exam, a student may:
- assess your muscle strength
- check your blood pressure
- check your pulse
- check your reflexes
- listen to your heart and lungs with a stethoscope
- look into your ears, eyes, and throat
- press on your abdomen looking for tenderness or swellings
- exam your genitals and/or rectum (only if you have expressed an interest in being involved in these teaching sessions)
Do You Have To Remove Your Clothing?
If students are only required to interview the patient, then you do not have to remove your clothing.
For scenarios that require you to undergo a physical exam, you would wear your underwear, with a hospital gown on top, open at the back. You would know beforehand when this would be expected.
Standardized Patient Training
Before you are interviewed by students, you undergo training by a Standardized Patient trainer, which may last up to 20 hours in total.
You'll learn how to simulate physical symptoms, your emotional state, and portray the role of a patient, family member, or other person.
If a case requires you to provide feedback and/or score student performance, you will be trained on that as well.
Before working with students, you are required to rehearse and successfully present your patient's character to the Standardized Patient trainer.
Do You Have To Be An Actor?
No, although many actors work as Standardized Patients. The focus is on providing students with an educational opportunity, not on performance or dramatic interpretation.
Do Students Know You're Not A Real Patient?
Yes. Students know they are seeing a Standardized Patient. However, they are instructed to treat you as though you are a real patient.
How Do You Know What To Say When A Student Interviews You?
To know what to say to the student interviewing you, you will learn the complete history of the patient, including:
- the patient's complaint
- the patient's past medical history
- details about the patient's life
- the patient's emotional state during the encounter
- how to move like the patient
- how to react to the physical exam
Standardized Patient Requirements
To be a Standardized Patient, you must:
- have intense concentration while you are interviewed and examined by multiple students in succession
- be able to respond exactly as a real patient would
- be able to maintain the patient's character, simulate the patient's physical condition during the interview and examination, and do so repeatedly in a consistent manner
- have excellent communication skills
- be able to discuss the encounter afterward with the student, by recalling the encounter precisely and describe carefully how the student's behavior impacted you as the patient
